Responsibilities
- Provide direct nursing care to residents per plan of care and physician orders
- Administer medications and treatments with accuracy and safety
- Monitor vital signs, assess changes in condition, and report to the supervising clinician
- Assist residents with activities of daily living and mobility needs
- Document assessments, interventions, and outcomes in the charting system
- Collaborate with the interdisciplinary team to support resident safety, comfort, and well-being
Requirements
- Active Licensed Practical Nurse license in Vermont
- Current Basic Life Support (BLS) certification
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to work evenings and nights
- Strong communication, observation, and documentation skills
- Ability to meet the physical and environmental demands of a long-term care setting
