Responsibilities
- Provide direct patient care to residents according to the plan of care under supervision.
- Administer medications and document administration in the resident chart.
- Monitor vital signs, assess changes, and report findings to the supervising clinician.
- Assist residents with activities of daily living and mobility as needed.
- Collaborate with the interdisciplinary team, including RNs, physicians, and therapy staff.
- Maintain accurate documentation, uphold safety and infection control standards, and adhere to facility policies.
Requirements
- Active Licensed Practical Nurse license in Vermont in good standing.
- Graduate of an accredited practical nursing program.
- 1 year of recent experience in a long-term care or similar setting preferred.
- Ability to work a flexible travel assignment and meet standard credentialing requirements.
- CPR/BLS certification preferred.
- Strong communication and teamwork skills; ability to document clearly and accurately.
