Responsibilities
- Provide direct nursing care to residents in a long-term care setting according to the plan of care.
- Administer prescribed medications and treatments under the supervision of a registered nurse.
- Monitor residents’ conditions, observe vital signs, and document changes promptly.
- Assist with activities of daily living and mobility, promoting resident safety and comfort.
- Collaborate with the interdisciplinary team to implement and adjust care plans.
- Maintain accurate documentation, ensure infection control practices, and uphold safety standards.
Requirements
- Licensed Practical Nurse license in good standing (state of New Hampshire) or eligible for licensure.
- Current BLS (American Heart Association) certification.
- 1+ year of experience in long-term care or skilled nursing settings preferred.
- Strong communication and documentation skills; ability to work collaboratively with the care team.
- Ability to work night shift and 12-hour shifts; reliable transportation may be required.
- Must be able to pass background check and drug screen per policy.
