Responsibilities
- Perform initial evaluations and ongoing assessments to determine therapy needs for residents in a geriatric rehabilitation setting.
- Develop and implement individualized treatment plans focusing on therapeutic exercise, gait training, balance, functional mobility, and activities of daily living.
- Deliver hands-on PT interventions and document progress, adjusting plans based on response and goals.
- Collaborate with an interdisciplinary team (nursing, OT, speech therapy, case management) to support discharge planning and optimal resident outcomes.
- Educate residents and families on home exercise programs, safety, fall prevention, and equipment use; ensure clear communication and goal tracking.
- Maintain accurate, timely documentation in the electronic health record and comply with regulatory and facility standards.
Requirements
- Active California Physical Therapy license or eligibility to obtain licensure.
- Ability to practice in a skilled nursing facility setting and work with an older adult population.
- Strong documentation, communication, and collaboration skills.
- Availability to start on 04/06/2026 and commit to a 13-week travel assignment with guaranteed hours.
- Demonstrated ability to work during a daytime, 5x8 schedule and to meet onboarding and credentialing requirements.
