Responsibilities
- Provide direct, compassionate nursing care to hospice patients in a home setting, including assessment, monitoring, symptom management, and wound care as ordered.
- Administer medications and treatments using the Pyxis system; document care in the Cerner electronic health record.
- Monitor patient status, interpret clinical data, identify changes, and escalate concerns to the care team as needed.
- Collaborate with physicians, nurse coordinators, social workers, and other team members to adjust plans of care and ensure patient comfort.
- Educate patients and families on disease progression, symptom management, safety, and medication administration; support families through care transitions.
- Ensure infection control, safety, and adherence to care plans and regulatory requirements.
Requirements
- Ability to work in a home healthcare/hospice setting.
- Experience with Cerner documentation and Pyxis medication system preferred.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to travel to patient homes and work the listed shift schedule.
- Adaptability to a fast-paced, patient-centered environment focused on comfort and quality of life.
