Responsibilities
- Provide direct patient care to residents on the post-acute care unit under RN/charge nurse supervision
- Administer prescribed medications and treatments per physician orders; monitor and document patient responses
- Perform or assist with phlebotomy, EKGs, and transports as permitted; manage IV pumps with appropriate certification
- Assist residents with activities of daily living and participate in restorative/rehabilitative activities
- Monitor for changes in condition, implement fall prevention and infection control measures, and report to the care team
- Maintain accurate documentation in the EPIC EMR and communicate changes to the care team

Requirements
- Active Vermont LPN license at time of submission
- BLS certification required
- EPIC EMR experience within the past 2 years
- Minimum 1 year of clinical experience; prior travel experience required
- Dementia-capable care certification preferred
- IV certification preferred
- Ability to work in a post-acute skilled nursing rehab setting; willingness to work 12-hour day shifts
- Comprehensive credentialing and background checks required (nationwide and county searches, FACIS3, VCIC) within specified timelines
- Reliable transportation required; public transit may not be accessible
- Must be able to work all holidays as assigned
