Responsibilities
- Provide direct nursing care to residents in a memory care setting under appropriate supervision
- Monitor resident conditions, administer medications and treatments as prescribed, and document responses
- Assess changes in status and communicate with the care team to adjust plans of care
- Support activities of daily living and implement individualized care plans with a person-centered approach
- Maintain accurate documentation, ensure safety, fall prevention, and infection control practices
- Collaborate with licensed staff, physicians, and interdisciplinary teams to coordinate ongoing care
Requirements
- Active LPN license in Vermont (or eligible by endorsement) and in good standing
- Experience in long-term care or memory care settings preferred
- Ability to work scheduled day and evening shifts
- Strong communication, observation, and documentation skills
- Reliable, professional, and able to work in a team-oriented, at-times fast-paced environment
