Responsibilities
- Provide direct patient care to residents in a long-term care setting under the supervision of an RN.
- Administer prescribed medications and document administration accurately per policy.
- Monitor vital signs, intake/output, and report changes to the supervising nurse.
- Assist residents with activities of daily living and mobility, prioritizing safety and comfort.
- Maintain accurate nursing documentation and participate in care planning and rounds; uphold infection control and safety procedures.
Requirements
- Active Vermont LPN license in good standing
- LPN diploma from an approved program
- CPR/BLS certification
- 2+ years of LPN experience in long-term care or a related setting preferred
- Ability to work day and evening shifts
- Willingness to travel for temporary assignments and work in a long-term care setting
