Responsibilities
- Conduct psychosocial assessments of patients and families to identify barriers to care and readiness for discharge.
- Develop and implement patient-centered discharge plans in collaboration with the patient, family, and the interprofessional team.
- Coordinate with hospital teams, home health services, social services, and community resources to arrange needed services, equipment, transportation, and supportive services.
- Provide counseling and ongoing support to patients and families, addressing emotional, social, and practical needs that affect health outcomes.
- Maintain accurate documentation in electronic medical records, including assessments, care plans, progress notes, and discharge summaries; ensure HIPAA compliance.
- Facilitate transitions from acute care to home by coordinating follow-up appointments and safety planning; identify and mitigate barriers to continuity of care.
- Participate in care conferences and discharge planning meetings; communicate changes in condition and care plan to all stakeholders.
- Monitor and adapt plans as patient needs evolve, documenting changes promptly.
Requirements
- Active New Jersey Licensed Social Worker (LSW)
- Discharge planning experience in a healthcare setting
- Acute hospital experience on Med/Surg and ED units
- Ability to work in a home healthcare travel assignment with weekend rotation as needed
- Strong communication, documentation, and interpersonal skills; proficient with electronic medical records
