Physical Therapist – Home Health in Mattoon, Illinois. This travel, temporary assignment begins 06/29/2026 for 13 weeks, with guaranteed 40 hours per week and a pay range of $2,213 to $2,283 per week. Shifts are 8:00 AM–4:30 PM (8-hour days) with the option to participate in 4x10s; no weekend shifts. Responsibilities
- Plan and execute rehabilitation programs to help patients regain physical function, using therapeutic exercise, neuromuscular re-education, modalities, gait training, and manual techniques
- Evaluate mobility, strength, and overall function; develop a Plan of Care with short- and long-term goals; provide a home exercise program and discharge instructions
- Document patient progress according to approved medical and facility documentation procedures; maintain patient records and ensure payer certifications and authorizations are in place
- Maintain open communication with therapy staff, department colleagues, and referral sources to coordinate care
- Supervise physical therapy assistants as appropriate and ensure consistent, quality care
- Engage in continuing education and apply current standards of practice to patient evaluations and interventions
Requirements
- Active Illinois Physical Therapist license (PT) – REQUIRED
- Active American Heart Association BLS certification – REQUIRED
- 1+ years of physical therapy experience; inpatient experience – REQUIRED
- Ability to work in an inpatient rehabilitation setting
- Two supervisory references completed with dates employed listed – REQUIRED
- Candidate must reside outside a 50-mile radius of Mattoon, IL (non-local requirement)
