Travel Physical Therapist Job
Physical Therapist travel assignment in Phillipsburg, New Jersey. Start date 06/22/2026 for 13 weeks. Weekly pay: $2,458–$2,511 per week. Guaranteed hours: 35 per week. Shift: 08:00–16:30, five days per week. Work setting is a long-term care environment (nursing home). This is a travel - temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Evaluate residents and develop comprehensive physical therapy plans of care to address mobility, strength, balance, and functional goals.
- Implement PT interventions including therapeutic exercise, gait and transfer training, range of motion, and functional activity training.
- Monitor progress, document evaluations and treatments, and adjust plans of care to meet goals and optimize outcomes.
- Collaborate with an interdisciplinary team (nursing, therapy colleagues, and other professionals) to coordinate care and discharge planning.
- Ensure patient safety, infection control, and adherence to regulatory and facility policies during therapy sessions.
- Maintain accurate, timely documentation of assessments, progress notes, and outcomes in the required systems.
Requirements
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Active Physical Therapist license in New Jersey or eligibility for licensure.
- Ability to work in a long-term care setting and care for geriatric populations.
- Strong communication and documentation skills; capable of teamwork within an interdisciplinary model.
- Prior physical therapy experience in a skilled nursing facility or geriatric rehabilitation setting is preferred.
- Ability to meet credentialing and regulatory requirements for travel assignments.
