Responsibilities
- Deliver direct resident care under the supervision of an RN, including routine assessments and observation of conditions.
- Administer medications and treatments as ordered, ensuring accurate documentation and adherence to policy.
- Monitor patient status, identify changes, and report findings promptly to the supervising nurse; respond to emergencies as appropriate.
- Assist residents with activities of daily living, infection control practices, and safety-focused care to support mobility and comfort.
- Maintain clear, accurate nursing notes and participate in care planning and interdisciplinary team communication.
- Collaborate with RNs, CNAs, and other clinicians to coordinate comprehensive resident care and ensure regulatory compliance.
Requirements
- Current Pennsylvania LPN license in good standing
- Experience in a long-term care setting preferred
- Ability to work a 40-hour week on a day shift (7:00 a.m. to 3:30 p.m.)
- Eligibility for travel/temporary assignments and compliance with credentialing and background checks
