Responsibilities
- Provide both direct and indirect skilled nursing care for adult and geriatric residents under the supervision of a Registered Nurse
- Collect ongoing data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team
- Contribute to development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve resident/family and healthcare team
- Reassess and revise the care plan based on resident outcomes; coordinate referrals to ensure comprehensive care
- Maintain current nursing knowledge and participate in in-service programs; mentor and orient staff when needed; demonstrate professional leadership in the clinical setting
Requirements
- Current NY LPN licensure
- Basic Life Support (BLS) certification
- Minimum 1 year of LPN experience in a long-term care setting
- Demonstrated ability to care for adult and geriatric residents
- Ability to work within a 24-hour minimum per 4 weeks and to cover 2 weekend shifts per month
- Ability to collaborate with the Interdisciplinary Team and operate under RN supervision
