Responsibilities
- Administer prescribed medications and treatments within the LPN scope and facility policies.
- Monitor residents’ conditions, perform routine assessments, and document findings; report changes to the supervising RN promptly.
- Assist residents with activities of daily living (ADLs), including mobility support, bathing, dressing, and personal care.
- Collaborate with licensed staff and the interdisciplinary care team to ensure comprehensive, person-centered care plans.
- Maintain accurate documentation in charting systems and ensure timely communication of any clinical concerns.
Requirements
- Active Vermont LPN license (state-specific) or current state-appropriate Licensed Practical Nurse license.
- CPR/BLS certification current.
- Prior experience in long-term care or skilled nursing settings preferred.
- Ability to work in a long-term care environment and adhere to regulations and facility protocols.
- Willingness to travel for a travel assignment and to begin on or near the specified start date.
