Responsibilities
- Deliver direct patient care to residents in a long-term care setting according to individualized care plans.
- Administer medications and treatments per physician orders; document nursing notes accurately.
- Monitor resident status, assess changes, and promptly report concerns to the supervising clinician; respond to call bells and emergencies as needed.
- Collaborate with an interdisciplinary team to implement care plans and ensure continuity of care and safety.
- Maintain thorough, compliant documentation in the electronic medical record and uphold infection control and safety standards.
Requirements
- Active LPN license in good standing in New Mexico
- LPN diploma from an accredited practical nursing program
- Current Basic Life Support (BLS) certification
- Ability to work 12-hour day shifts with a 36-hour weekly schedule
- Prior experience in long-term care or skilled nursing is preferred
- Strong communication, documentation, and teamwork skills
