Responsibilities
- Assess residents to determine occupational therapy needs and goals

- Document evaluations, progress notes, and outcomes in the medical record; update plans as needed
- Collaborate with the interdisciplinary rehabilitation team to coordinate care and discharge planning
- Educate residents and caregivers on home programs and safety strategies to support independence
Requirements
- Ability to work effectively in a long-term care setting
- Strong assessment, documentation, and communication skills
- Experience collaborating with interdisciplinary teams
- Availability to commit to a 13-week assignment with 35 guaranteed hours per week
- Proficiency with standard rehabilitation documentation systems
