Responsibilities
- Provide skilled nursing care in patients’ homes, assessing needs in stable and emergent situations; document findings and coordinate with the interdisciplinary team, patient, family, and referring agency
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care; adjust as patient status changes
- Collect data on physical, psychological, social, spiritual and cultural factors and integrate this information into the POC
- Maintain up-to-date patient records; ensure visits, contacts, problems, plans, actions and goals are clearly documented and updated promptly
- Apply infection control measures to protect staff and patients; educate patients and families on care, medications, home safety, and disease/injury management; promote self-care practices
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure safe, consistent, high-quality service
- Assess and reassess pain; implement appropriate pain management strategies; educate patient and family about pain control
- Coordinate referrals to appropriate agencies when needs are identified; monitor third-party payer requirements for compliance
- Participate in on-call duties as assigned; serve as a liaison to patients/families, team members, and physicians
- Attend interdisciplinary team meetings and other required discussions to support cohesive care planning
Requirements
- Active Colorado RN license; current CPR certification
- Valid driver’s license and reliable transportation; must maintain auto insurance
- 1–2 years of RN clinical care experience; 2+ years of home health experience preferred
- OASIS experience required; Homecare Homebase software experience required
- ADN or Nursing Diploma program graduate required; BSN preferred
- CHPN certification preferred; ability to work in home health settings and travel locally
