Responsibilities
- Conduct PT evaluations and develop individualized plans of care for residents in a long-term care setting
- Implement evidence-based interventions to improve mobility, strength, balance, gait, and functional independence
- Reassess progress regularly, update goals, and modify treatment plans as needed
- Document assessments, treatments, and outcomes in the patient record per policy and regulatory requirements
- Collaborate with nursing staff, physicians, and the broader care team to coordinate resident care
- Participate in interdisciplinary care planning and discharge planning as appropriate
- Ensure safety, infection control, and compliance with facility policies and licensing requirements
Requirements
- Active Physical Therapist license or eligibility to practice; licensure in the applicable state (NH) or eligible for transfer
- Ability to work in a long-term care setting and manage a 13-week travel assignment
- Strong clinical assessment, documentation, and communication skills
- Capacity to collaborate with a multidisciplinary team and families
- Commitment to resident safety, quality of care, and regulatory compliance
- Availability for a 5x8 day shift pattern
