Responsibilities
- Administer prescribed medications and monitor responses, following physician orders and facility protocols
- Conduct regular resident assessments, document findings, and communicate changes to the care team
- Develop, implement, and adjust individualized care plans in collaboration with licensed colleagues and interdisciplinary staff
- Provide direct patient care including activities of daily living support, wound care, and symptom management within scope of practice
- Monitor vital signs, recognize clinical changes, respond to emergencies, and escalate concerns promptly
- Maintain accurate documentation in electronic health records, ensure infection control practices, and uphold patient privacy and safety standards
Requirements
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Graduation from an accredited practical nursing program; current licensure required
- Basic Life Support (BLS) certification
- Experience in or exposure to long-term care or skilled nursing settings preferred
- Ability to work effectively in diverse long-term care environments and with multidisciplinary teams
- Strong communication and teamwork skills, with a patient-centered approach
