Responsibilities
- Administer medications and treatments per physician orders and facility policy
- Monitor residents’ conditions, perform vital signs assessments, and report changes to the supervising RN
- Document care provided and update electronic health records accurately
- Assist residents with activities of daily living, including mobility, grooming, and personal care
- Maintain infection control practices and ensure resident safety
- Collaborate with the interdisciplinary team to implement individualized care plans
- Respond to call lights promptly and provide comfort and safety interventions as needed
Requirements
- Active LPN license in New York (or eligible to practice)
- Graduation from an accredited practical nursing program
- Prior experience in long-term care or skilled nursing settings preferred
- Proficiency with basic charting/electronic health records and medication administration
- Ability to work night shifts and adapt to a fast-paced environment
- Strong communication and teamwork skills
