Responsibilities
- Administer medications and monitor residents’ responses, reporting changes per plan of care
- Perform routine nursing assessments, document findings, and communicate with the care team
- Provide basic wound care and dressing changes as prescribed; monitor skin integrity
- Monitor vital signs, chart progress, and escalate concerns to the supervising RN/charge nurse
- Assist residents with activities of daily living and mobility, ensuring safety and dignity
- Document care accurately and maintain compliance with applicable policies and procedures
Requirements
- Active Licensed Practical Nurse license in New York
- Current CPR/BLS certification or ability to obtain
- 1+ year of nursing experience in a long-term care or related setting preferred
- Ability to work night shift (7:00 pm–7:00 am) and adapt to a nocturnal schedule
- Strong communication, observation, and documentation skills
- Ability to perform essential LPN duties in a long-term care environment
