This travel PT role is a home healthcare - hospice assignment available through Healthforce Healthcare LLC. The position requires delivering skilled physical therapy services in a patient home setting, with a focus on functional independence and safety.

Responsibilities
- Actively monitors and amends patient visit schedules to maximize productivity and meet patient needs, including rescheduling and load balancing; communicates changes to supervisor.
- Perform comprehensive PT assessments (including OASIS and PT) covering psychosocial, safety, and risk factors; document per federal/state regulations and APTA guidelines.
- Develop and update a plan of care to promote optimal functional independence and safety; create patient-specific goals and ensure interventions advance toward those goals.
- Plan and provide skilled PT treatment, educate patients and caregivers, and assess progress at each visit to ensure quality of care and regulatory compliance.
- Coordinate patient care as a Care Manager or team member with physicians, nurses, caregivers, and other rehab services; maintain clear communication in the clinical record.
- Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with regulations and standards of care; ensure appropriate delegation and documentation.
- Complete and coordinate required paperwork to reflect care provided and outcomes; support billing processes and address insurance coverage limitations as needed; notify supervisor of issues promptly.
- Maintain ongoing professional development and participate in in-service training; contribute to QA and process improvements where appropriate.
Requirements
- Active Physical Therapist license in Vermont (in good standing) or ability to obtain as required
- Doctor of Physical Therapy (DPT) degree from an accredited program
- 1+ year of physical therapy experience preferred in home health or hospice settings; clinically versatile candidates considered
- Ability to conduct and document OASIS and PT assessments in accordance with agency, state, and federal guidelines
- Demonstrated ability to work effectively in a home health/hospice environment; reliable transportation to patient visits
- Strong communication and teamwork skills; ability to supervise PTAs, students, and home care aides as needed
