Responsibilities
- Provide skilled nursing care to hospice patients in a home-based setting during night shifts.
- Assess patient status, monitor vital signs, manage symptoms, and respond promptly to changes in condition.
- Administer prescribed medications and therapies per orders; document in the electronic health record; coordinate with the care team.
- Develop and update individualized care plans in collaboration with physicians, therapists, social workers, and family members.
- Educate patients and families on disease progression, symptom management, medication safety, and end-of-life care.
- Ensure patient safety, uphold nursing standards, complete timely documentation, and maintain regulatory and agency compliance.
Requirements
- Active California RN license (state of practice)
- CPR/BLS required; ACLS required; HLFS required
- Minimum 2 years of RN experience in the submitted unit specialty within the last 3 months
- BSN preferred
- Acute care facility experience preferred
- Ability to work night shifts in a home hospice/home healthcare setting
