Responsibilities
- Deliver coordinated nursing care for an assigned patient group using the nursing process and established standards.
- Assess patient condition, document findings, monitor vitals, and respond to changes in status with appropriate interventions.
- Develop and implement a care plan, coordinate with the interdisciplinary team, and ensure safe, patient-centered outcomes.
- Administer medications and treatments per orders, monitor for adverse effects, and adjust care as needed under physician direction.
- Educate patients and family members, provide discharge planning support, and promote understanding of home hospice needs.
- Directly supervise and delegate tasks to licensed and unlicensed members of the nursing team while maintaining compliance with regulations and facility policies.
- Document care and outcomes in the EPIC EMR, ensuring accurate, timely charting and communication across care teams.
Requirements
- Licenses/Certifications: Current registered nurse license in the state of practice (California); Basic Life Support (BLS); Advanced Cardiovascular Life Support (ACLS); Hospital Fire and Life Safety (HLFS) certification from an approved vendor as required by policy.
- Education: Bachelor’s Degree in Nursing (BSN) preferred.
- Experience: At least two years of recent nursing experience; ICU or high-acuity care experience preferred.
- Additional: EPIC EMR experience required; ability to perform in a home healthcare - hospice setting; valid for the assignment location; willing and able to travel as needed for patient care coverage.
