Responsibilities
- Provide comprehensive case-management–focused patient care by developing, implementing, revising, and reporting care plans.
- Act as liaison among the patient, family, nurse, physicians, and the multidisciplinary team to optimize outcomes.
- Collaborate with the healthcare team to identify barriers, coordinate resources, and advocate for patient goals.
- Document care plans, progress notes, and outcomes; communicate changes to the team and stakeholders.
- Support transitions of care and discharge planning as appropriate.
- Serve as a clinical consultant to team members on specific patient items.
Requirements
- Active, unrestricted Registered Nurse license in California
- Experience in case management or care coordination preferred
- Strong communication and documentation skills; ability to work with patients, families, and multiple disciplines
- Ability to work in a home healthcare – hospice setting; comfortable with travel as part of the role
- Knowledge of care planning, patient advocacy, and outcome tracking
- Eligibility to work in the United States and comply with travel assignment requirements
