Responsibilities
- Conduct physical therapy evaluations and develop individualized treatment plans for residents in the skilled nursing setting.
- Implement and monitor therapeutic programs focusing on mobility, gait, balance, strength, ROM, and activities of daily living.
- Administer therapeutic modalities and supervise progress toward functional goals; document outcomes and adjust plans as needed.
- Collaborate with an interdisciplinary team to coordinate care, participate in care planning, and assist with discharge planning.
- Ensure patient safety, maintain infection control standards, and comply with relevant regulations and documentation requirements.
Requirements
- Active California Physical Therapist license
- Doctor of Physical Therapy (DPT) degree or equivalent
- Experience in skilled nursing or geriatric rehabilitation preferred
- Availability for a 14-week travel assignment beginning 04/20/2026
- Ability to meet 36 hours per week on a 5x8 shift
- U.S. work authorization
