Responsibilities
- Perform in-home assessments to identify functional impairments and therapy needs.
- Develop and implement individualized plans of care focused on safety, self-care, and mobility.
- Provide hands-on interventions addressing ADLs, home safety, and patient-specific goals.
- Educate patients and caregivers on adaptive equipment and techniques to support independence.
- Collaborate with physical therapists, nurses, aides, and physicians for coordinated care.
- Document progress and adjust care plans in response to clinical changes; support smooth transitions and long-term functional wellness.
Requirements
- Licensed Occupational Therapist (OTR/L) in New Mexico.
- Minimum of 1 year of clinical experience; home health experience preferred.
- Strong problem-solving, communication, and time-management skills.
- Able to work independently in patients’ homes.
- Experience with EMR systems and mobile charting tools.
- Comfortable serving patients within a defined service area around Albuquerque (service radius up to 45 miles).
