Responsibilities
- Provide excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program.
- Serve as a liaison among the patient, family, nurse, physicians, multidisciplinary team, and the patient’s healthcare benefactor to optimize outcomes.
- Act as a consultant to the care team on specific patient items and care pathways.
- Coordinate a continuum of care for a defined population from pre-admission through post-discharge.
- Ensure patients and families progress efficiently through the course of hospitalization and transition planning.

Requirements
- Active Registered Nurse license in California.
- Background in case management or clinical care coordination within a healthcare setting.
- Ability to work effectively in a multidisciplinary team and communicate clearly with patients and families.
- Strong organizational skills for care coordination and discharge planning.
- Willingness to participate in a travel assignment and work the listed day-shift schedule.
