Responsibilities
- Assess residents’ rehabilitation needs and develop individualized PT plans to improve mobility, strength, balance, and functional independence.
- Deliver evidence-based physical therapy interventions within a skilled nursing facility setting, following physician orders and facility policies.
- Document patient progress, modify treatment plans as needed, and communicate outcomes to the care team.
- Collaborate with nursing staff, physicians, and families to coordinate care and support discharge planning.
- Monitor safety during therapy sessions, maintain infection control practices, and ensure patient comfort and dignity during all treatments.
- Maintain timely, accurate charting and adhere to regulatory and professional standards for patient care.
Requirements
- Active Physical Therapist license in good standing for practice in the United States; state-specific licensure for the assignment required.
- Degree from an accredited physical therapy program (DPT preferred; master’s degree acceptable) with eligibility for licensure.
- Ability to work effectively in a skilled nursing facility environment and with older adult populations.
- Strong communication and documentation skills; ability to collaborate with a multidisciplinary team.
- Willingness to comply with credentialing and onboarding processes for travel assignments.
