- Maintain accurate medical records and charting in the designated system
- Collaborate with the interdisciplinary team to implement and update care plans
Requirements
- Active LPN license in Colorado
- CPR/BLS certification
- Prior experience in skilled nursing or long-term care preferred
- Ability to work night shifts and adapt to a fast-paced SNF environment
- Strong communication, documentation, and teamwork skills
- Proficiency with basic computer charting and electronic health records
