Responsibilities
- Provide direct patient care to residents in a long-term care setting, delivering safe, evidence-based nursing support.
- Administer prescribed medications and perform treatments according to care plans and facility policy.
- Monitor resident conditions, interpret changes in status, and document observations accurately in the chart.
- Collaborate with the interdisciplinary team (including CNAs, RNs, physicians, and therapists) to implement person-centered care plans.
- Assist with admissions, transfers, and discharges; promote resident safety and comfort during routine and urgent situations.
- Ensure compliance with regulatory, infection control, and documentation standards; maintain clean, organized patient areas.

Requirements
- Active Licensed Practical Nurse license in good standing; current Vermont LPN licensure preferred or eligible for practice in Vermont.
- Ability to work evenings and nights; willingness to participate in a traveler schedule.
- Experience in a long-term care or skilled nursing environment is preferred.
- Strong communication and teamwork skills; reliable, detail-oriented, and able to follow care plans accurately.
- Must be able to perform essential nursing duties and maintain patient safety in a geriatric setting.
- Eligible to work in the United States and meet assignment onboarding requirements.
