Responsibilities
- Provide basic nursing care to residents in a long-term care setting as outlined in the plan of care.
- Monitor vital signs, observe changes in condition, and document findings accurately.
- Administer prescribed medications and perform routine nursing procedures in accordance with policy and scope of practice.
- Assist residents with activities of daily living, mobility, hygiene, and nutritional needs; promote independence where possible.
- Collaborate with the supervising RN and care team to implement and update care plans; communicate changes promptly.
- Maintain thorough, compliant documentation in the charting system; emphasize safety, infection control, and resident dignity.
Requirements
- Active Licensed Practical Nurse license (New York) or eligible to practice in New York.
- Ability to perform duties in a long-term care setting with compassion and professionalism.
- Availability to work the day shift (12-hour shifts: 7:00 AM–7:00 PM).
- Prior experience in long-term care or specialized geriatric settings preferred.
- Strong communication and teamwork skills; commitment to resident-centered care.
