Responsibilities
- Provide direct nursing care to residents per individualized care plans and physician orders.
- Administer prescribed medications and document administration accurately.
- Monitor and assess patient status, vital signs, and changes; report findings to the supervising RN.
- Collaborate with the interdisciplinary team to plan, implement, and document care; update progress notes.
- Assist residents with activities of daily living and mobility; provide comfort and dignity in care.
- Maintain a safe, clean, and compliant environment; support infection control, safety, and quality of care standards.
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Graduation from an approved practical nursing program
- CPR/BLS certification
- 1 year of nursing experience; long-term care experience preferred
- Ability to work night shifts and function effectively in a LTC setting
- Strong communication and documentation skills
