Responsibilities
- Provide direct and indirect skilled nursing care to residents under the supervision of a Registered Nurse.
- Collect ongoing data on residents’ physical and psychosocial status and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant plan of care, involving residents and families, to achieve desired outcomes.
- Review and revise the plan of care as needed based on resident outcomes; coordinate referrals to ensure comprehensive care.
- Maintain current nursing knowledge and participate in orientation and continuing education activities.
- Collaborate with other nursing staff, physicians, and ancillary staff to promote safety and quality of care; demonstrate professional practice leadership under supervision.
Requirements
- Active NY Licensed Practical Nurse licensure
- BLS certification (through AHA or RQI)
- Minimum 1 year of experience as an LPN
- Experience as an LPN in a long-term care setting
- Ability to provide care to adult and geriatric residents
- Ability to work nights and weekends/holidays as required
- Demonstrated ability to assess resident data and communicate effectively with the care team
- Knowledge of growth and development principles for adult and geriatric patients
