Responsibilities
- Conduct initial assessments and develop individualized OT treatment plans for residents in a SNF setting.
- Deliver evidence-based occupational therapy interventions focused on activities of daily living, mobility, cognition, and adaptive equipment training.
- Monitor progress, document outcomes, and maintain accurate records in accordance with regulatory and facility requirements.
- Collaborate with an interdisciplinary team (including nursing, therapy disciplines, social work, and discharge planning) to optimize resident outcomes and transitions.
- Adapt therapy plans as needed based on patient response, goals, and safety considerations; provide education to residents and families.
- Ensure safe, functional therapy sessions and uphold professional standards and compliance with facility policies.
Requirements
- Active Occupational Therapist license in Maryland, or eligibility to obtain licensure in Maryland.
- Ability to work in a skilled nursing facility environment and participate in a 13-week travel assignment with guaranteed hours.
- Strong communication and collaboration skills to work with an interdisciplinary care team.
- Ability to document and maintain patient records accurately and in a timely manner.
- General OT competencies applicable to a SNF setting, with focus on ADLs, mobility, and adaptive equipment.
