Responsibilities
- Conduct PT evaluations and provide treatment for patients in outpatient/home health settings, with weekend inpatient duties as scheduled.
- Develop and implement individualized care plans focusing on functional mobility, gait and balance, strength, endurance, and activity tolerance.
- Document assessments, plan of care, progress notes, and discharge summaries in the authorized electronic medical record; communicate with the care team and supervising clinicians.
- Educate patients and families on home exercise programs, safety during transfers, pain management, and strategies to maintain progress after discharge.
- Collaborate with interdisciplinary team members to optimize outcomes and coordinate plans across settings; participate in occasional care conferences as needed.
- Maintain current licensure and certifications (CPR/BLS) and adhere to state regulations, clinical policies, and facility guidelines; participate in ongoing professional development.
Requirements
- Active Physical Therapy license in the state of practice (California) required
- CPR or Basic Life Support (BLS) certification from an approved vendor required
- 3–5 years of physical therapy experience preferred
- Women’s health experience and/or certification (PRPC or WCS) or active pursuit of certification; willingness to discuss at interview
- Ability to work in outpatient/home health settings with occasional inpatient duties; weekend availability every 5–7 weeks; no on-call requirement
- Education: Physical Therapy degree from an accredited program (licensure in state required)
