Responsibilities
- Conduct patient evaluations and develop individualized plans of care for physical therapy needs.
- Implement therapeutic interventions including therapeutic exercise, gait training, and modality use as appropriate.
- Monitor patient progress, document outcomes, and adjust treatment plans in coordination with the care team.
- Ensure patient safety, proper use of equipment, and adherence to safety and infection control protocols.
- Maintain timely, accurate documentation and comply with billing and coding procedures per program guidelines.
- Participate in discharge planning and provide patient education to support functional recovery and independence.
Requirements
- Active physical therapy license in Illinois or eligibility to obtain licensure
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program
- Ability to work days on a 12-week travel assignment
- Strong documentation, communication, and teamwork skills
- Ability to meet onboarding and credentialing requirements and follow an established billing/procedural workflow
