Registered Nurse – Case Management in Sitka, Alaska. Travel - Temporary Assignment start date 10/12/2026 for 13 weeks, with guaranteed 40 hours per week and a 5x8 Days shift. Responsibilities
- Perform comprehensive patient assessments and develop individualized plans of care.
- Administer medications and treatments safely and accurately per provider orders and policy; document in the electronic health record.
- Monitor patient condition, recognize changes, and initiate appropriate interventions or escalation to the provider.
- Provide patient and family education related to care, medications, and discharge needs; coordinate care with providers and interdisciplinary teams.
- Facilitate referrals, follow-up care, and continuity of services; document outcomes accurately and timely.
- Maintain infection control practices, regulatory compliance, and participate in quality and performance improvement activities; may float to other departments as needed.
Requirements
- Current, unrestricted RN license in Alaska or another U.S. state; must have an active Alaska license at time of submission.
- Direct Hire must have applied for an Alaska nursing license before start date; agency staff must have an active Alaska license.
- Graduate of an accredited school of nursing; Bachelor’s degree preferred; high school diploma or equivalent required.
- Basic Life Support (BLS) certification; Case Management certification preferred.
- Experience in ambulatory/outpatient nursing and care coordination; ability to work effectively in an outpatient clinic setting.
