Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients per established standards and the nursing process.
- Assess, plan, implement, and evaluate care; document findings accurately and timely.
- Supervise and direct activities of multiple levels of nursing staff; delegate tasks appropriately.
- Coordinate care with multiple disciplines; communicate effectively with patients, families, and team members.
- Administer medications and treatments per orders; monitor for adverse effects and respond to changes in patient condition.
- Maintain safe, patient-centered care and compliance with hospice/home health policies and procedures.
Requirements
- Current RN license to practice in California.
- BLS certification required; ACLS recommended for ER-related duties.
- ER or acute care nursing experience; travel or hospice exposure a plus.
- Ability to work in a home healthcare/hospice setting and adapt to variable environments.
- Strong assessment, critical thinking, and interpersonal communication skills.
