Responsibilities
- Conduct initial and ongoing PT evaluations to determine patient needs, goals, and appropriate treatment plans.
- Develop and implement individualized treatment programs, including therapeutic exercise, gait training, balance work, and functional techniques.
- Monitor patient progress, document outcomes, and adjust treatment plans to achieve goals.
- Collaborate with an interdisciplinary care team (nurses, physicians, therapists, and aides) to coordinate care and discharge planning.
- Ensure patient safety, proper documentation, and adherence to infection control and facility policies.
- Communicate progress and recommendations clearly to patients, families, and team members.
Requirements
- Active Physical Therapist license in Maryland or eligibility for MD licensure
- Graduation from an accredited physical therapy program (DDPT/DPT or equivalent) or current eligibility to practice
- Ability to work effectively in a skilled nursing facility setting
- Willingness to participate in a 13-week travel assignment
- Strong communication and documentation skills
