Responsibilities
- Provide direct nursing care to residents in the long-term care setting, including monitoring health status and administering prescribed medications and treatments under appropriate supervision
- Perform ongoing assessments, document changes in resident conditions, and escalate issues to the supervising RN or physician per facility policy
- Assist residents with activities of daily living, mobility, and personal hygiene, while promoting comfort, dignity, and independence
- Implement and update individualized care plans, ensuring adherence to safety, infection control, and quality-of-care standards
- Accurately document care provided, medication administration, and resident responses in the electronic medical record
- Collaborate with the interdisciplinary team, including CNAs, RNs, and other staff, to coordinate care and ensure smooth transitions between shifts
Requirements
- Active Licensed Practical Nurse license in New Mexico
- Graduation from an approved Practical Nursing program (diploma or equivalent)
- Current CPR/BLS certification
- Previous nursing experience in long-term care or skilled nursing preferred
- Ability to work 3x12 night shifts and adapt to a night-shift schedule
- Effective communication and teamwork skills; ability to document clearly and work well in a fast-paced care environment
