Responsibilities
- Provide direct nursing care to residents in a long-term care setting, including monitoring health status and supporting activities of daily living.
- Administer medications and treatments as prescribed, following policy, charting accurately, and reporting any discrepancies or changes to the supervising nurse.
- Perform routine assessments, monitor vitals, manage wounds, and assist with mobility, transfers, and comfort measures.
- Maintain accurate documentation in the electronic health record, ensure timely communication with the interdisciplinary team, and follow individualized care plans.
- Promote resident safety, implement fall-prevention strategies, and respond effectively to on-call needs and changes in condition.
- Collaborate with therapists, physicians, and families to coordinate care, provide education, and support discharge or transition planning when appropriate.
Requirements
- Active Vermont LPN license or eligibility for VT licensure
- Graduation from an approved practical nursing program
- Prior experience in long-term care or geriatric care preferred
- Ability to perform professional nursing duties in a long-term care setting and adhere to care plans
- Strong communication and teamwork skills; comfortable working in a travel assignment with flexible scheduling
