Responsibilities
- Provide excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program.
- Act as a liaison between the patient, family, nurse, physicians, multidisciplinary team and the patient’s healthcare benefactor to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items.
- Coordinate a continuum of care for a defined population of patients from pre-admission through post-discharge.
- Ensure patients and families proceed efficiently through the course of hospitalization.
Requirements
- Experience in case management or care coordination in a hospital setting is preferred.
- Strong communication and interpersonal skills to collaborate with patients, families and multidisciplinary teams.
- Ability to coordinate transitions of care from pre-admission to post-discharge.
- Ability to document and report case management activities accurately.
- Must be able to work a daytime 40-hour per week schedule in Glendale, CA.
