Licensed Practical Nurse needed for a travel assignment in a long-term care setting in South Burlington, VT, United States. Responsibilities
- Provide basic nursing care to residents under supervision, including activities of daily living, vital signs, and medication administration per license and facility policy.
- Monitor residents’ conditions, identify changes, and promptly escalate to the supervising RN; document findings clearly in the chart.
- Administer prescribed medications and treatments in accordance with medication administration records and safety protocols.
- Assist with care planning and deliver support for mobility, nutrition, hydration, and daily living activities to promote well-being.
- Maintain patient safety, adhere to infection control practices, and keep accurate nursing records; communicate effectively with the care team.
- Collaborate with an interdisciplinary team to deliver individualized, high-quality care within a long-term care setting.
Requirements
- Active Vermont Licensed Practical Nurse license
- Ability to work in a long-term care setting providing direct patient care
- Proficient in vital signs, medication administration, MAR/eMAR documentation, and basic nursing procedures
- Strong written and verbal communication skills; ability to document clearly and work with a team
- Flexibility to cover Day and/or Evening shifts and participate in a travel assignment as needed
