Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents; develop and implement individualized treatment plans aligned with resident goals.
- Deliver therapy focused on mobility, gait, balance, strength, and activities of daily living; monitor progress and adjust plans as needed.
- Document therapy findings, progress, and discharge plans in accordance with regulatory requirements and facility policies.
- Collaborate with an interdisciplinary team including nursing, physicians, and therapy staff to coordinate care and plan discharges.
- Promote safety through proper transfer techniques, fall prevention, and patient/family education on home programs and functional goals.
- Ensure compliance with infection control, safety standards, and professional practice guidelines during all therapy sessions.
Requirements
- Active physical therapy license in good standing in the United States or eligible for licensure in the applicable jurisdiction.
- Experience in skilled nursing or geriatric rehabilitation settings preferred.
- Ability to commit to a 13-week travel assignment with guaranteed hours.
- Strong documentation and communication skills; comfortable working within an interdisciplinary team.
- Eligibility to begin on 08/03/2026 and operate within a travel assignment framework.
