Responsibilities
- Conduct initial evaluations and develop individualized physical therapy plans of care within scope of practice for residents in the long-term care setting.
- Deliver therapeutic interventions including therapeutic exercise, gait and balance training, strength training, ROM, and mobility tasks; tailor activities to residents’ goals and safety needs.
- Monitor progress, document sessions and outcomes in the electronic health record, and communicate findings with the interdisciplinary care team.
- Reassess and adjust treatment plans as residents progress; participate in discharge planning and transitional care as appropriate.
- Educate residents and families on exercises, safety strategies, and adaptive equipment; ensure infection control and safety protocols are followed.
Requirements
- Active Montana Physical Therapist license or eligibility for MT licensure
- Graduation from an accredited physical therapy program (Doctor of Physical Therapy preferred)
- Ability to practice in a geriatric/long-term care setting and collaborate with an interdisciplinary team
- Strong documentation and communication skills; proven ability to work with diverse resident populations
- Availability to work 36 hours per week on dayshift for the 13-week assignment
