Responsibilities
- Administer prescribed medications and treatments within scope of practice and facility policy
- Conduct resident assessments and monitor changes in condition; document findings promptly
- Collaborate with registered nurses, certified nursing assistants, and other care team members to implement and adjust individualized care plans
- Assist residents with activities of daily living and provide basic nursing interventions as directed
- Maintain accurate and timely nursing documentation; communicate changes to the care team and families as appropriate
- Ensure infection control practices, safe patient handling, and a clean, safe care environment; respond to changes in patient condition and escalate as needed
Requirements
- Active Licensed Practical Nurse license appropriate for New Mexico (or eligible for licensure in NM)
- CPR/BLS certification
- Experience in a long-term care or skilled nursing setting is preferred
- Ability to work a 3x12.5-hour day shift schedule and meet guaranteed hours
- Strong communication, documentation, and teamwork skills
- Ability to pass background check and drug screening as required
