Travel Registered Nurse – Home Health in Hermiston, Oregon. Start date 04/13/2026 for a 13-week assignment. Weekly pay range is $2,231–$2,355 with guaranteed 40 hours per week. Shift: Day shift, 5x8 schedule (8:00 a.m.–4:30 p.m.) with weekend rotation. Responsibilities
- Assess, delegate, coordinate, and evaluate the nursing care provided in the home setting.
- Initiate the Plan of Care and necessary revisions; ensure overall quality of care for the patient.
- Conduct ongoing assessments of patients’ physical needs and related symptoms; develop individualized care plans aligned with patient goals.
- Communicate observations and treatment plans clearly with patients, families, caregivers, and providers; participate in interdisciplinary team meetings.
- Provide education to patients, families, and caregivers on disease and symptom management; maintain thorough documentation of assessments, plans, treatments, and outcomes.
- Advocate for the patient and coordinate services across disciplines to support safe, effective care; adhere to established standards of care and regulatory requirements.
Requirements
- License: Current RN license in the State of Oregon.
- Education: Graduate of an accredited school of professional nursing; BSN preferred.
- Experience: One year of continuous RN experience in an acute care setting; Home Health experience and/or community health experience preferred.
- Certifications: BLS is required; CPR certification also noted.
- Driving/ mobility: Must maintain a valid Oregon driver’s license, current auto insurance, and have own automobile for home visits.
- Other: Ability to work in a home health setting, with effective communication and documentation skills; willingness to participate in required education related to program standards.
