Responsibilities
- Provide direct, resident-centered nursing care to adults in a LTC setting under the supervision of an RN; respond promptly to changes in condition and urgent needs.
- Administer prescribed medications and document administration accurately, following policy, state regulations, and safety protocols.
- Monitor vital signs, perform routine assessments, and communicate changes to the supervising clinician in a timely manner.
- Assist with activities of daily living (mobility, bathing, dressing, nutrition, hygiene) to support resident comfort and independence.
- Maintain a clean, safe environment; uphold infection prevention, safety standards, and wound care as directed.
- Document care accurately in charts and care plans; participate in handoffs and interdisciplinary rounds to support coordinated care.
Requirements
- Active Licensed Practical Nurse (LPN) license in New Mexico (or ability to practice per state requirements for this assignment)
- Current BLS/CPR certification
- Prior experience in a long-term care or skilled nursing setting preferred
- Ability to work a 3x12-hour shift pattern and perform duties in a local Santa Fe assignment
- Strong communication, teamwork, and documentation skills
- Must pass background check and drug screening
