Travel Physical Therapist Job
Physical Therapist, travel assignment in Nashua, New Hampshire. Start date: May 25, 2026. Duration: 13 weeks. Weekly pay range: $2,433–$2,564 per week with guaranteed hours of 40 per week. Shift: 5x8 days, 08:00–16:30. Setting: long-term care (nursing home). This is a temporary travel position through Healthforce Healthcare LLC.

Responsibilities
- Perform comprehensive PT evaluations for residents, document findings, and help establish individualized goals in collaboration with the interdisciplinary team.
- Develop and implement treatment plans targeting mobility, strength, balance, gait, ROM, and functional independence.
- Provide skilled interventions such as therapeutic exercise, gait training, balance activities, and modality use as ordered.
- Monitor progress, reassess plans of care regularly, adjust goals as needed, and prepare discharge or transfer summaries when appropriate.
- Maintain accurate, timely documentation in accordance with facility policies, payer guidelines, and HIPAA requirements.
- Communicate effectively with residents, families, and the care team to ensure safe, high-quality care and smooth care transitions.
Requirements
- Current PT license to practice in New Hampshire or eligibility for licensure
- Ability to work in a long-term care setting and follow facility policies
- Strong assessment, documentation, and communication skills; ability to collaborate with a multidisciplinary team
- Ability to work a 5x8 day schedule (08:00–16:30) and meet guaranteed hours
- Commitment to patient safety, quality of care, and professional behavior
